Providence PACE (ElderPlace) program (Program of All-Inclusive care for the Elderly) is an innovative program that helps older adults remain independent in the community for as long as possible.

 

We specialize in serving the 55+, Medicaid population who are medically complex and require the services of a caregiver. We operate from an Interdisciplinary model of care and coordinate  all aspects of our participants heath care and social services.

 

We provide all transportation to and from medical appointments and our Adult Day Health activities program. We also provide durable medical equipment, prescribed medications, incontinence supplies and have onsite physical and occupational therapy at our clinics.

 

Providence ElderPlace is open Mon-Fri 8am-5pm with 24 hr on-call staff.
