Nursing Services | 24-hour skilled care and rehabilitation services.

Social Services | Assisting patients and their families during transitional periods, providing essential information, managing requests and concerns, and helping in care planning and discharge planning.

Rehabilitation | Individualized programs to reach goals of increased independence and ability. Patient families are also encouraged to participate in therapy sessions, caregiver education, and training.

Long Term and End of Life Care | Care focusing on differing populations of individuals--many patients require several days or weeks of medical care and therapy before they can safely return home or to a lower level of care.

The African American Elders Program, run by Catholic Community Services, serve older B/AA who are home bound, hard to serve, low income, and have chronic and/or severe disabilities, living with dementia and related disorders, and are at high risk of premature institutionalization. The program specializes in providing culturally competent and sensitive care specific to B/AA that have lived in a high crime environment and with a lifetime battling discrimination and racism.

The Care Coordinators (CC's) are Master's-prepared Geriatric Social Workers who identify with the long-term effects of racism. They provide assistance to clients who have deficits of one activity of daily living (ADL) and/or two Instrumental (IADL's) deficits; and are unable to access services on their own; and do not have anyone to help them assess these services.

Care Coordination includes conducting a comprehensive assessment that focuses on cognitive and functional areas, evaluation of social circumstances, and resources. The assessment also includes medical diagnosis, nutritional concerns, emotional issues, medication management, safety, support, caregiver needs and continuous support with healthcare providers. This assessment leads to the development of a service plan and implementation. The care coordinators continue to follow-up and monitor the outcomes and status of the service plan for optimal outcomes and to amend service plans as needed.

Follow-up activities includes: home visits, telephone, and electronic communications with clients or their representatives.

The National Resource Center on LGBT Aging is the country's first and only technical assistance resource center aimed at improving the quality of services and supports offered to lesbian, gay, bisexual and/or transgender older adults. Established in 2010 through a federal grant from the U.S. Department of Health and Human Services, the National Resource Center on LGBT Aging provides training, technical assistance and educational resources to aging providers, LGBT organizations and LGBT older adults. The center is led by SAGE, in collaboration with 18 leading organizations from around the country.

The Northwest LGBT Senior Care Providers Network is a network of senior care providers working together to provide advocacy and quality of care for the LGBT seniors of Washington State. Members offer their expertise in fields that include, but are not limited to, attorneys, community services and senior centers, fiduciaries, financial planning, government agencies, home care agencies, home health agencies, housing providers, information services, insurance providers, moving services, and pharmacy services.

Monthly meetings, with guest speakers, cover a wide array of topics focusing on LGBT elders today and tomorrow.

Sound Generations has six community partner sites and ten programs that connect people to various food, security, transportation, health and wellness, and assistance services throughout King County. A network of community connections provides multiple services for seniors and adults with disabilities and those who care for them, including telephone, in-person, and written information and assistance; a network of senior centers; the Meals on Wheels and grocery delivery programs; minor home repair services, including making disability modifications; and arranging for transportation. Information is available on legal topics, e.g., end-of-life planning and an elder law clinic. We offer complimentary health and wellness programs and serve as health insurance benefits advisors for King County.

PAEP or Pacific Asian Empowerment Program was born out of a grassroots demand by Filipinos, Tongans, Hmong, Mien, Laotians, Samoan, Polynesian volunteers, and clients demanding changes from oppression and lack of understanding by larger groups and agencies that had not been culturally sensitive to their heritage and needs.

PAEP and its Senior Nutrition Congregate Meal Program addresses many important issues such as diminishing and alleviating hunger, isolation, loneliness, depression, malnutrition, obesity, inactivity, and sedentary lifestyles.ย  When the seniors come to the meal sites they socialize, exercise, listen to music, sing, dance, eat a healthy and tasty meal using ingredients similar to the foods they were used to eat back home.ย  The seniors receive good, helpful information, peer counselling, information and referral, food bank donated items.ย  They sometimes play bingo, watch the news and discuss issues and current events with others. ย We are helping families and seniors in the Seattle, King County area to live healthier, happier and more independent lives, thus contributing to an improved community with a healthier and more productive population.
