As Geriatric Case Managers, we work with individuals/ families to find the best choices for their individual needs. We provide excellent oversight, care and resources to our clients. We assist with doctor appointments, transportation, medication management, selection and logistics of new housing and so much more. We also have a wide variety of vendors who excel in their professions who can assist the client. Examples include moving specialist, meal delivery, transportation services, home organizers, in home companions, medical assistance, lawyers, financial consultants, housekeepers, pet care and so much more. In addition, our work often includes educating clients about their choices for housing such as retirement livings, assisted livings, memory care, Adult Family homes, and hospice.
