As Geriatric Case Managers, we work with individuals/ families to find the best choices for their individual needs. We provide excellent oversight, care and resources to our clients. We assist with doctor appointments, transportation, medication management, selection and logistics of new housing and so much more. We also have a wide variety of vendors who excel in their professions who can assist the client.
