Location Information
Description:
Our Care Coordination services provide bi-lingual and bi-cultural short-term case management assistance to immigrant senior and adults with disabilities who are low-income, LES/NES and with limited or no supporting network which makes them vulnerable to institutional placement. The individuals we serve are primarily transitioning from the hospital, an assisted living situation or group home back to the larger community. Priority is given to clients with acute or chronic illness such as COPD, congestive heart failure, or diabetes.
